Dr Rachel Yeoh is a consultant geriatrician who provides practical, compassionate care for older adults with complex health needs. She focuses on helping patients maintain independence, clarity, and quality of life through thoughtful, individualised medical care.

A graduate of Monash University, Dr Yeoh completed her specialist training in geriatric medicine at Eastern Health and Alfred Health. She is a Fellow of the Royal Australasian College of Physicians and a member of the Australian and New Zealand Society for Geriatric Medicine.

Dr Yeoh has broad experience across hospital and community settings and has particular expertise in memory assessment, falls, delirium, medication review, and the management of multiple medical conditions. She is known for her clear communication style and collaborative approach, working closely with patients and families to support informed, shared decisions.
